North Arlington Pediatric's Medical Home

The staff at NAP is working with families to create a medical home for children who have special health care needs (a chronic condition lasting more than one year). The medical home is a relationship between you and the health care professionals who care for your child. A medical home is an accessible, family-centered, comprehensive, continuous, compassionate, and culturally effective way for NAP to partner with you in the care of your child. As part of the medical home, we have formed a team of practice staff and parents to help us promote and provide quality care for children with special health needs.

At North Arlington Pediatrics we accommodate children and families at appointment time and have developed a process to assist us in identifying families' special needs. When you call to schedule an office visit, our staff will ask if your child has any special needs that we should be aware of. For example:

  • Will you need extra time at this appointment to discuss your child's needs with the doctor or other office staff?
  • Will you need help getting into the office building or the exam room?
  • Will you need special accommodations in the waiting room or in the exam room? ( For example, does your child feel better waiting in the exam room or the waiting room where there is less stimulation?)
  • Would you like to meet with our Care Coordinator during this visit to discuss your child's health insurance, coordination of benefits, educational issues, or referrals for specialty care, therapies, or other services?

Care Coordination Services

Care Coordination is a process that links children with special health care needs and their families to needed services and resources. Our staff will:

  • Serve as an advocate for children and their families.
  • Coordinate coverage with private insurance, Medicaid, and All Kids including:
    • Document diagnosis codes.
    • Provide copies of medical records.
    • Provide letters of medical necessity.
    • Coordinate referrals for specialty care.
    • Coordinate authorization for services and supplies.
  • Answer phone calls from families with questions regarding services and resources.
  • Provide families with information on referrals and resources.
  • Assist families in organizing and tracking medical information about their child.
  • Link families with other families for support.
  • Provide authorized information to schools and Early Intervention programs.
  • Help with transition issues.
  • Facilitate the development and maintenance of a written Care Plan.
